HCA ATHLETIC PROCEDURES
HCA 2017-2018 SPORTS SIGN-UP
To complete your registration for the 2017-18 sports programs you must fill out and return the following papers to the Athletic office by August 1, 2016. If papers are not on file in the Athletic office prior to the start of a sport, the student is not eligible to participate.
August 1st is the deadline for you to bring or mail all paperwork to the Athletic office and have signed up for the sports you would like to play this year.
Go to “Sports Offered 2017-2018” on the wepage under Athletics and you will see what sports are available this coming year. NOTE: If there are 8th grade tryouts available for a HS team, it is noted on the form. You will be contacted by the Athletic Director or Coach if they would like your son/daughter to “play up” on a HS team.
We will be having our annual two sports meetings for our middle school and high school programs. We would like all of you playing any sport this entire school year to attend one of the meetings listed below. If you are an 8th grader, planning on playing or trying out for a HS team in any sport (see 8th grade tryouts column on 'Sports & Teams Offered' page), you need to attend the HS Sports meeting:
High School Sports Meeting = Tuesday, August 1st @ 6:30pm in gym
Middle School Sports Meeting = Tuesday, August 22nd @ 6:30pm in gym
Paper work listed below is to be turned into the Athletic office BEFORE any student may participate in a practice/game for ANY sport. These are required by the Minnesota State High School League (MSHSL) and must be followed.
Item #1 = PRIVIT / e-PPE (Electronic Pre-Participation Evaluation ) Registration or Update
HCA asks all MS & HS families to fill out an e-PPE (Electronic Pre-Participation Evaluation) on-line for each student. e-PPE is an internet based, comprehensive and detailed risk-assessment questionnaire that each student athlete with parent assistance completes. It follows strict international privacy guidelines and creates a summary report for physicians, coaches and administration. This is a cutting edge program under the Minnesota State High School League (MSHSL) that Heritage is piloting with other schools.
To access the electronic form, go to heritageweb.org and under the page EAGLES ATHLETICS, select “Statement & Forms” and, and then Sports Registration (e-PPE)
Complete the Questionnaire, following the prompts and answer all the questions until your status shows 'Complete'. This process will take about 20-35 minutes to complete depending on your family. Both student and parent should be available to help with specific questions that need to be answered. Immunization history will need to be accessible. If you need to stop before finishing the questionnaire, you can start again later. Once all sections (background/heart/etc.) are at 100% complete you can proceed to the next steps. Please remember to select the sports (for the current school year) that your child will be participating in. We ask that all students, MS and HS, sign up accordingly. Just click/check which sports you want to participate in. Click 'Done' at the bottom of the screen when completed. This is our official sign-up procedure, and helps us in budgeting and hiring coaches.
NOTE: if you have more than one child playing sports you need to update the information for your other children or "Add New Family Member" from the Home page under Manage Family, and proceed with the prompts for the next child.
CURRENT FAMILY or ATHLETE? Click on “LOG IN,” enter your email address and password, click “LOG IN” and then click “CONTINUE” on the WELCOME MESSAGE Page.
If you need to make updates to your PERSONAL DETAILS OR E-PPE QUESTIONNAIRE, please make those updates at this time. You need to make sure you are at 100% completion in order to access PRINT DOCUMENTS. Please remember to select the sports (for the current school year) that your child will be participating in. We ask that all students, MS and HS, sign up accordingly. Just click/check which sports you want to participate in. Click 'Done' at the bottom of the screen when completed. This is our official sign-up procedure, and helps us in budgeting and hiring coaches.
Item #2 = PRINT "Clearance Form" in PRIVIT / e-PPE—MUST BE TURNED INTO HCA ATHLETIC OFFICE!
Once your information is at 100% completion in e-PPE, you can now access PRINT DOCUMENTS. Print the “Clearance Form” and make sure that both the student and parent sign this form. Each student playing in a sport must have this signed form. This form is only good for the current school year. You DO NOT need a physician's signature on this page.
Item #3 = Under EAGLES ATHLETICS, Statements & Forms, PRINT the MSHSL Sports Qualifying Physical Examination Clearance Form in PRIVIT / e-PPE OR use Physical Examination Form from your Health Care Provider—MUST BE TURNED INTO HCA ATHLETIC OFFICE!
Every athlete (MS or HS) must have a physical on file at HCA. HCA asks that all MS students playing on a middle school team must have a physical clearance form signed/dated from a Physician on file. The student playing on a high school team must have a physical clearance form on file that is dated within the last 3 years to comply with MSHSL.
Check Powerschool for your current Medical Exam Date: When you log into Powerschool, select Grades & Attendance and scroll to the bottom of the page to find your students Medical Exam Date. Please verify that your child(s) physical is in the required timeframe.
Item #4 = Under EAGLES ATHLETICS, Statements & Forms, PRINT page 7 (MSHSL Eligibility Statement) and page 8 (MSHSL Annual Sports Health Questionnaire) of the MSHSL Eligibility Brochure—MUST BE TURNED INTO HCA ATHLETIC OFFICE! Every athlete in 7th grade and up playing on a HS team must have the last two pages on file in the Athletic office. This form must be signed by both the parent and student and is only good for the current school year.
